Job summary

Derbyshire Healthcare NHS Foundation Trust in Derby is recruiting an experienced Mental Health Nurse to join the Dementia Rapid Response Team. You will lead a MDT, deliver person-centred, non-pharmacological care to older adults with dementia, and help prevent hospital admission by enabling safe, earlier discharge.

Based in Derby, the team covers Derby City and South Derbyshire, operates across 7 days with flexible shifts and a 12-week rolling roster.

Qualifications

• Knowledge of Mental Health Act and Capacity Act requirements.
• Willingness and ability to travel across the catchment area as required.
• Mentorship/SSSA training or willingness to complete training.

Responsibilities

• Lead nurse responsibilities within a multi-disciplinary team and complex case management.
• Support delivery of a flexible, responsive dementia care service in the community.
• Travel independently across the catchment area and potentially outside it.
