Derbyshire Healthcare NHS Foundation Trust in Derby invites Mental Health Nurses to join the Dementia Rapid Response Team. We support older adults living with dementia in crisis, using person-centred, non-pharmacological care delivered in the community across Derby City and South Derbyshire.

Responsabilidades

• Manage complex cases as part of a wide multidisciplinary team (MDT) including Occupational Therapy, Physiotherapy, Nurses, Dementia Specialist Support Workers, Psychology and a Consultant Psychiatrist.
• Be clinically responsible for supporting the delivery of a flexible, responsive local dementia service.
• Travel independently across the catchment area and occasionally outside of it when required.
• Work a shift pattern that facilitates operation 7 days per week.
• Undertake the Duty role: triaging referrals, allocating visits, preparing and delivering handovers, ensuring colleagues are safe and well.
• Complete initial patient assessments and apply the Mental Capacity Act and Mental Health Act when required.
• Assess risk and formulate safety plans; develop person-centred care plans focusing on non-pharmacological interventions.
• Carry out in-reach to inpatient wards and visit patients in their own homes and in 24‑hour care settings.
• Utilise the EPR to ensure excellent documentation of patient assessment and treatment.
