Job summary

Essex Partnership University NHS Foundation Trust is recruiting for a role within the Crisis Resolution Home Treatment Team. The post involves assessing adults and older adults with mental health problems, planning and reviewing care programmes, and working within a multi-disciplinary team across wards and primary care.

Candidates should hold a professional qualification in occupational therapy, social work or mental health nursing and have completed ASYE, with Social Work England registration.

Qualifications

• Completed ASYE (Assessed and Supported Year in Employment)

Responsibilities

• Carry out holistic assessments, plan, deliver and evaluate interventions with accurate records
• Participate in initial assessments to decide suitability for Crisis Resolution Home Treatment Service
• Build therapeutic relationships with clients and families, working non-judgementally
• Carry out clinical risk assessment and monitor mental state
• Share information with health and social care agencies through case conferences and written reviews
• Identify clients with violent or potentially violent behaviour and arrange joint visits when needed
• Develop follow-up care with relatives and carers on admission and discharge
• Report breaches of professional conduct as required
• Implement and develop Trust policies in collaboration with the line manager
• Work flexibly across the service, including on-call rota and clinical work
• Participate in team meetings and research or trials
• Maintain high standards of care in an integrated multi-disciplinary environment
• Maintain professional boundaries and use professional judgement confidently
• Inform about additional qualifications or experience related to dementia as desired

Education
