Job summary

You will work closely with clients and families, maintain confidential information, and participate in on-call arrangements, case conferences, and collaborative risk management to ensure safe and effective community care.

Qualifications

• Excellent communication and inter-agency liaison skills.
• Ability to assess risk and monitor mental state.
• Experience in home treatment or crisis services is desirable.

Responsibilities

• To be aware of values, beliefs and attitudes to provide non-judgmental, objective client care.
• Establish therapeutic relationships with clients and their families.
• Ensure effective and confidential communication between health and social care agencies; participate in case conferences and reviews.
• Be aware of clinical risk management and monitor clients’ mental state to evaluate risk to self or others.
• Identify and manage high-risk clients with behavioural problems in the community, organising joint visits if necessary.
• Advise managers on breaches of professional conduct as required by regulatory codes.
• Liaise with line managers to implement Essex Partnership NHS Foundation Trust policies and procedures.
• Work flexibly across the Crisis Resolution Home Treatment Service within the Trust.
• Take on clinical work commensurate with experience in liaison with supervisors and senior staff.
• Contribute to service quality improvement and arrange cover for Home Treatment and Crisis Services as needed.
• Participate in on-call arrangements for Crisis Resolution Home Treatment Service.
