Job summary

Essex Partnership University NHS Foundation Trust is seeking a social worker to join the Crisis Resolution Home Treatment Team, covering adults and older adults with mental health problems referred to the service. You will assess care needs, plan, deliver and review care programmes while ensuring joined-up working across wards and primary care.

The role is part of a multidisciplinary team, includes an on-call rota, and requires access to a vehicle for work.

Qualifications

• UK registered social worker status required.
• Must have completed preceptorship period after qualifying.
• Experience in mental health and urgent care settings is advantageous.

Responsibilities

• Carry out holistic assessment of clients' problems and needs, and plan, deliver and evaluate interventions.
• Assess referrals for suitability for the crisis service and liaise with referring agencies on alternative support.
• Take part in team and clinical meetings, introducing new models of care and research or trials.
• Build therapeutic relationships with clients and their families.
• Carry out clinical risk assessment and monitor clients' mental state.
