Job summary

Devon Partnership NHS Trust in Torbay is seeking a Community Support Worker to join the Crisis Resolution Home Treatment Team (CRHTT). The role focuses on delivering a 24/7 community-based service as an alternative to hospital admission and gatekeeping for inpatient beds.

You will be part of an integrated MDT, providing interventions for diverse mental health needs in the community and carrying out initial follow-ups after hospital discharge when required.

Qualifications

• Experience in mental health support is desirable.
• Ability to work collaboratively within a multidisciplinary team (MDT).
• Strong interpersonal skills and crisis support abilities.

Responsibilities

• Provide responsive, person-centred support to individuals experiencing mental health crisis in the community.
• Work collaboratively with clinicians, social workers and peer support workers.
• Deliver practical and emotional support to facilitate recovery, safety and wellbeing.
• Assist in crisis de-escalation and safety planning, both face-to-face and via telephone.
• Support service users to access housing, benefits and community resources.
