Job summary

Devon Partnership NHS Trust is seeking a Community Support Worker for the North Devon CRHTT. The role is 37.5 hours across 5 days, providing community-based crisis resolution and home treatment as an alternative to hospital admission.

You will be part of an MDT, delivering care plans, crisis interventions and ongoing support, with a focus on recovery and safety in the community. Applicants should be able to travel within the trust area and work evenings/weekends as needed.

Qualifications

• Previous care experience in mental health or caring roles.
• Experience delivering crisis interventions.
• Safeguarding awareness and MDT experience.

Responsibilities

• Provide responsive, person-centred support to individuals experiencing mental health crisis in the community.
• Work collaboratively with multidisciplinary teams including clinicians and social workers.
• Deliver practical and emotional support to promote recovery, safety and wellbeing.
• Assist in crisis de-escalation and safety planning, face-to-face and via phone.
• Help service users access housing, benefits and community resources.
• Maintain accurate records per Trust policies.
• Participate in meetings, supervision and training.
• Promote equality, diversity and inclusion.
• Work flexibly including evenings and weekends.
