Qualifications

• Experience of working with individuals with multiple and complex needs.
• Experience delivering support that empowers change.
• Understanding of Homelessness Reduction Act 2018 and its approach to preventing homelessness.
• Ability to assess need and deliver effective support.

Responsibilities

• Provide crisis intervention and holistic support to adults experiencing homelessness.
• Work to prevent homelessness and improve health and wellbeing.
• Develop Personalised Housing Plans (PHPs) with the homelessness assessment outreach team.
• Identify support needs and help access services (DAAT, mental health, counselling).
• Support people with complex needs to access housing and related services.
