Job summary

Bupa is seeking a Connected Care Lead at New Victoria Hospital in Kingston upon Thames to coordinate patient pathways and MDTs across private care services. This role focuses on seamless referral to treatment, liaising with consultants, clinical teams and Bupa Insurance, and driving service improvements.

The successful candidate will manage multiple priorities in a fast-paced healthcare setting, ensuring high-quality patient journeys and strong stakeholder relationships within the Connected Care

Qualifications

• Experience in healthcare administration, patient coordination or MDT coordination.
• Excellent organisational and workload management skills.
• Strong communication and stakeholder management abilities.

Responsibilities

• Coordinate referrals, bookings and patient pathways across multiple Bupa services.
• Act as liaison between patients, consultants, clinical teams, diagnostics services and Bupa Insurance.
• Monitor patient progression and resolve pathway delays.
• Coordinate and administer up to three MDTs.
• Organise MDT meetings, prepare documentation and communicate outcomes.
• Support pathway performance monitoring and service improvement initiatives.
• Build relationships with internal and external stakeholders.
• Contribute to development and growth of Connected Care services.
