Job summary

CPSL Mind is recruiting for a Connect To Work Triage Co-ordinator based in Peterborough with a hybrid working option. The role involves triaging expressions of interest for the Connect to Work programme, coordinating between referrers and partner providers, and ensuring individuals receive guidance to enter, return to, or stay in work.

The successful candidate will provide initial contact support, oversee eligibility pre-assessments, and manage referrals through to specialist employment

Responsibilities

• Support data collection for reporting against KPIs.

Salary: £27,113.00 per annum

Salary Scale Point: 15

Hours: 37.5 hours per week, Monday to Friday

Based: Peterborough with hybrid working option

About the Service

The Connect to Work Triage Coordinator is a key role, coordinating activity between referrers and partner providers to ensure individuals receive support and guidance to support them to enter, return to, or stay in work. This role is responsible for triaging Expressions of Interest for the Connect to Work programme, delivered in partnership with Cambridgeshire County Council and other specialist providers. This role will:

• Provide the first point of contact and support for individuals referring in
• Oversee completion of pre-assessment of eligibility against defined criteria
• Ensure the smooth handover of eligible referrals through to specialist employment services
• Connect ineligible referrals with an appropriate local service, maintaining a strong understanding of CPSL Mind services, local groups and other local service providers
• Support the collection of data and impact stories required for reporting against key performance indicators
• Take general enquiries

Key Criteria

• Demonstrable ability to connect with individuals and staff with empathy, compassion and understanding in relation to their unique challenges and needs
• Strong verbal and written communication skills are essential, with the ability to clearly articulate ideas and instructions
• Proven ability to work collaboratively and build strong professional relationships with a diverse range of stakeholders, including individuals, employers and healthcare providers
