Job summary

CCH Group is hiring a Complex Community Nurse to deliver high-quality home-based care across the Taunton, Bristol and Cornwall regions. You’ll balance clinical leadership with practical hands-on nursing in clients’ homes, building strong relationships while planning your own diary.

The role focuses on complex care needs, allowing nurses to use advanced clinical skills outside hospital settings, with a Monday–Friday schedule and on-call duties to support patients in the community.

Qualifications

• Registered Adult and Children's Nurse (NMC Registered).
• Experience in Critical Care (ICU/HDU), Complex Care, Respiratory, Neurosciences, Spinal Injuries or Community Nursing.
• Ability to manage clients with complex clinical needs.

Responsibilities

• Conduct comprehensive clinical assessments for new clients leaving hospital.
• Develop and review person-centred care plans.
• Deliver hands-on training and competency assessments for care professionals.
