Job summary

Qualifications

• Nursing qualifications and registration with the NMC (implied by role).
• Ability to undertake routine assessments and patient reviews.
• Experience or exposure to complex care and chronic conditions is desirable.

Responsibilities

• Undertake routine assessments, patient reviews and monitoring.
• Support people with complex long-term conditions to live well at home.
• Phlebotomy, blood pressure and weight monitoring; CGM set-up/review.
• Administer medications including insulin and provide self-management education.
