Continuity of Care Services Ltd. is seeking a Complex Care Supervisor in Maidstone to oversee complex care packages across Kent.

The role combines supervisory duties with hands-on clinical support in a 40-hour week, including office-based work, community visits, and clinical shifts with alternating weekends. The ideal candidate will have a minimum of one year experience with complex health needs, QCF level 3 Health and Social Care, and a full UK driving licence.

Qualifications

• Excellent communication, organisational, and problem-solving skills.
• Full UK driving licence and your own car is essential for this role.
• Attend mandatory fully funded training.

Responsibilities

• Collaborate with Care Manager to setup care packages and introduce new staff to clients.
• Maintain familiarity with the complex needs of each client to ensure continuity of care.
• Supervise, shadow, manage competency assessments, observations, and mentor care assistants.
• Work with the care coordinator to maintain shift cover within the care package team.
• Perform clinical duties of personal care, medication administration, meal preparation and light household duties.
• Participate in the on-call rota, shared with the wider team.
