Job summary

Liverpool, Merseyside-based Nurse Case Manager – Complex Care role, full-time, Mon-Fri 9-5, no weekends. You will oversee complex care packages and ensure clinical excellence across services. Requires active NMC pin and experience in high dependency or critical care; community tracheostomy/ventilation experience is a plus.

Qualifications

• NMC pin required and current nursing registration.
• Experience in high dependency or critical care settings is essential.
• Experience with tracheostomy and ventilation in a community setting is highly desirable.
• Strong communication and organizational skills; IT proficiency.
• Understanding of Care Quality Commission (CQC) regulations.

Responsibilities

• Clinical management of complex care packages.
• Develop and review individualized care plans and risk assessments.
• Lead clinical training sessions and conduct competency assessments.
• Attend MDT meetings and engage with commissioning groups.
• Perform audits on care notes and medication records to ensure quality and compliance.
