Job summary

Aberdeen Cyrenians is seeking caring Home Care Support Practitioners to provide person-led care in service users’ homes in Aberdeen. You will follow individual plans, document notes, and work within a supportive team to deliver compassionate care and maintain safety and dignity.

The role offers flexible hours, extensive training, and the chance to develop in a values-driven charity environment. Successful candidates will have SVQ3 or be willing to work towards it, plus strong communication

Qualifications

• Must have N/SVQ3 or equivalent or willingness to work towards SVQ3.
• Literacy and numeracy to maintain paperwork and reports.
• Able to use a smartphone app and Word.
• Proven track record in a similar role.

Responsibilities

• Lead and participate in high-quality person-led home care aligned with care plans and legislation.
• Communicate effectively with service users, families and multi-disciplinary teams, and keep care notes.
• Provide domiciliary care including personal and continence care, medication management, and meals.
• Maintain safety and promote dignity, independence and rights of service users.
• Support other Practitioners and follow trauma-informed approaches.
• Use PASS for service user case management and keep accurate records.
• Work alone and in teams, comply with regulations, and uphold organisational standards.
• Promote health, safety and environmental standards within the service.
