• The community palliative care team are looking for an enthusiastic, motivated registered nurse to join Thundersley/Benfleet palliative care team as a band 6.
• The post holder will hold a caseload of end of life patients and work as a key member of the Palliative Care Team delivering care alongside nurses and therapists to end of life patients.
• The post holder will work to reduce all inappropriate admissions to secondary care through the use of advanced care planning and practical high quality end of life care.
• To become part of the community palliative care team to provide holistic care for patients with life limiting conditions.
