Job summary

Waythrough is seeking a compassionate Social Isolation Support Coordinator to join our Community Services team in Newton Aycliffe. You will work one-to-one with adults 55+ experiencing loneliness or isolation, helping them set goals, access activities and improve wellbeing.

You will shape a new role, build local partnerships, and demonstrate the impact of this service to commissioners, contributing to better outcomes for local people and stronger community ties.

Qualifications

• Experience with vulnerable adults or older people.
• Understanding of loneliness and social isolation.
• Experience assessing needs and developing support plans.
• Knowledge of safeguarding principles and procedures.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage own caseload.
• Full driving licence and access to a vehicle.
• Desirable: health and social care qualification or related field.
• Experience in social prescribing or wellbeing support is beneficial.

Responsibilities

• Work with individuals to understand their needs, strengths, goals and barriers.
• Develop personalised support plans to improve wellbeing, confidence and independence.
• Provide practical and emotional support to those experiencing loneliness or isolation.
• Help individuals access local community groups, activities and services.
• Accompany individuals to appointments or activities to build confidence.
• Provide information and signposting to health, social care, housing and welfare services.
• Build relationships with local organisations and partners.
• Monitor progress and maintain records.
