Job summary

Waythrough is seeking a compassionate Social Isolation Support Coordinator to join the Community Services team in Newton Aycliffe. You will support adults aged 55+ facing loneliness and isolation, helping them build confidence, access services and form meaningful social connections.

This role involves developing personalised support plans, working independently to manage a caseload, and collaborating with local organisations to improve wellbeing. Closing Date: 2026-11-02.

Qualifications

• Experience working with vulnerable adults, older people or wellbeing needs.
• Understanding causes and impact of loneliness and social isolation.
• Ability to assess needs and develop support plans.
• Knowledge of safeguarding principles and procedures.

Responsibilities

• Work with individuals to understand needs, goals and barriers.
• Develop personalised support plans to improve wellbeing and independence.
• Provide practical and emotional support for loneliness and anxiety.
• Assist individuals to access local groups, activities and services.
• Accompany to initial appointments or activities to build confidence.
• Provide information, advice and signposting to health, social care and housing services.
• Build relationships with local organisations and community partners.
• Monitor progress and maintain records of outcomes.
• Recognise safeguarding concerns and follow procedures.
