At Waythrough, we're looking for a compassionate and motivated Social Isolation Support Coordinator to join our Community Services team. This is an exciting opportunity to join a new role supporting adults aged 55 and over who may be experiencing loneliness, social isolation, poor mental wellbeing or difficulties accessing their local community.
You'll work directly with individuals on a one-to-one basis, helping them build confidence, improve their wellbeing and develop meaningful social connections. You'll support people to identify their goals, explore opportunities within their communities and access the services and activities that can make a real difference to their lives.
As this is a new role, you'll have the opportunity to shape how the service develops, build relationships with local organisations and demonstrate the positive impact this support can have. Your work will help us show commissioners the value of the service and its potential to improve outcomes for local people.
You’ll work independently, managing your own caseload while building a strong network of local connections to help people access the support they need.
We're looking for someone who is approachable, empathetic and confident in building relationships with people from a range of backgrounds. You'll understand the impact loneliness and social isolation can have on someone's life and be passionate about helping people feel valued, connected and empowered.
You will have:
A relevant qualification in health and social care, community development, counselling, advice and guidance, or a related field would be desirable.
Experience in social prescribing, community navigation or wellbeing support would also be beneficial. However, we welcome applications from people with transferable skills who can demonstrate the ability to engage effectively with individuals and build positive, trusting relationships.
We value the people who make a difference every day. Alongside meaningful work, you'll enjoy a comprehensive benefits package:
