Job summary

Waythrough is seeking a compassionate Social Isolation Support Coordinator to join our Community Services team, focusing on adults 55+ who may be experiencing loneliness, isolation or wellbeing challenges. You will work one-to-one to build confidence, improve wellbeing and help people access local activities and services.

You will shape the service as it develops, build relationships with local organisations, monitor progress and maintain records to demonstrate impact to commissioners.

Qualifications

• Experience working with vulnerable adults, older people or individuals with wellbeing needs.
• Knowledge of safeguarding principles and procedures.
• Experience assessing individual needs and developing support plans.
• Excellent communication and interpersonal skills.
• The ability to work independently and manage your own caseload.
• A full driving licence and access to a vehicle, as required for the role.

Responsibilities

• Work with individuals to understand their needs, strengths, goals and the barriers they face.
• Develop personalised support plans focused on improving wellbeing, confidence and independence.
• Provide practical and emotional support to people experiencing loneliness, anxiety, low confidence or social isolation.
• Help individuals access local community groups, activities, volunteering opportunities and other support services.
• Accompany people to initial appointments or community activities where appropriate, helping them feel confident taking the next steps.
• Provide information, advice and signposting to relevant health, social care, housing, welfare and community services.
• Build positive working relationships with local organisations and community partners.
• Monitor progress, review support plans and maintain accurate records of outcomes.
• Recognise safeguarding concerns and follow the appropriate procedures to help keep people safe.
