Job summary

Derbyshire Community Health Services NHSFT in Swadlincote is seeking a compassionate Therapy Support Worker to join the Community Therapy Team. You will support rehabilitation programmes, help patients with exercises and daily living activities, and work closely with physiotherapists, occupational therapists and other health professionals in community settings.

The role offers development opportunities, including potential Apprenticeship and 7-day service coverage 08:30-16:30.

Qualifications

• Experience of working with patients in health, social care or community settings.
• Excellent communication and interpersonal skills.
• Compassionate, caring and patient-centred approach.
• Ability to work independently and as part of a multidisciplinary team.
• Willingness to undertake training and develop competencies.
• Access to transport for travel between care settings.

Responsibilities

• Support rehabilitation programmes prescribed by registered therapists.
• Assist patients with therapeutic exercises and activities of daily living.
• Provide, fit and monitor therapeutic equipment and aids.
• Monitor and report changes in patients' conditions.
• Communicate effectively with patients, families and the multidisciplinary team.
• Maintain accurate clinical records and contribute to patient reviews.
• Promote patient independence, wellbeing and recovery.
