Job summary

Derbyshire Community Health Services NHSFT is seeking an Advanced Therapy Support Worker to join the Pathway 2 Therapy Team at Bennerley Fields, Ilkeston, with duties across Ilkeston and Ripley Hospitals. The role supports timely discharge within a seven-day service, working in a multidisciplinary team including Occupational Therapists and Physiotherapists.

The post involves assessment, care planning, and liaison with senior therapy staff, plus travel between care settings.

Qualifications

• NVQ Level 4 in health or social care or equivalent.
• English Language and Maths GCSE or equivalent.
• IT skills and experience using SystmOne/TPP.

Responsibilities

• Assess patients and review care plans in collaboration with the MDT.
• Communicate health needs and treatment plans to patients and carers.
• Maintain accurate health records using System One/TPP and report changes in condition.
