Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking an Advanced Therapy Support Worker to join community teams in Chesterfield and North East Derbyshire. The role includes initial patient assessments, creating care plans, and supporting therapy goals in patients' homes, with weekend roster duties and travel between locations.

Part-time posts (0.8 and 1.0 WTE) require flexible working, strong communication, and the ability to work independently while liaising with therapists,

Qualifications

• Experience in rehabilitation, physiotherapy or occupational therapy support work desirable.
• Access to and ability to use transport as travel between and across the locality will be required.

Responsibilities

• Carry out initial assessment and reviews of patients and create patient-led care plans to meet therapy goals.
• Liaise with other specialist services within the Trust and with nursing and social care teams.
• Communicate information to patients and carers regarding health needs and treatment plans.
• Report any change in patient condition and communicate with the integrated care team.
• Support senior therapy staff in ongoing development of services.
• Maintain and record accurate health records using System One/TPP in line with Trust policies.
• Organise and prioritise day-to-day work, work independently and adhere to Lone Working policies, and delegate tasks when needed.
