Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking a compassionate Therapy Support Worker to join our Community Therapy Team in Swadlincote. You will support rehabilitation, assist with exercises and mobility, and help patients regain independence in their own homes as part of a multidisciplinary team.

The role involves delivering high-quality patient care across community settings and includes opportunities to apply for the Advanced Therapy Support Worker Apprenticeship.

Qualifications

• Has experience of working with patients or service users in a health, social care or community setting.

• Can work both independently and as part of a multidisciplinary team.
• Has good organisational and time management skills.

Responsibilities

• Assist patients with therapeutic exercises, mobility practice and activities of daily living.
• Provide, fit and monitor therapeutic equipment and aids.
• Monitor and report changes in patients' conditions to the relevant healthcare professional.
• Communicate effectively with patients, families and members of the multidisciplinary team.
• Maintain accurate clinical records and contribute to patient reviews.
• Promote patient independence, wellbeing and recovery.

Working alongside Physiotherapists, Occupational Therapists and other health professionals, you will play a key role in supporting patients to achieve their rehabilitation goals and maximise their independence. This is an exciting opportunity for someone who is passionate about delivering high-quality patient care and enjoys working as part of a multidisciplinary team across community settings. As you develop in DCHS there will also be the opportunity for you to apply for theAdvanced Therapy Support Worker Apprenticeship. The service provides cover 7 day per week, 08:30-16:30 providing care to our community Therapy patients. As a Therapy Support Worker, you will:
