Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking a compassionate Therapy Support Worker to join our Community Therapy Team in Swadlincote. You will help patients achieve rehabilitation goals and maximise independence in home and community settings.

You will work alongside physiotherapists and occupational therapists, delivering prescribed rehabilitation programmes and supporting daily living activities.

Qualifications

• Experience with patients or service users in health, social care or community settings.
• Excellent communication and interpersonal skills.
• Compassionate, caring and committed to patient-centred care.
• Ability to work independently and as part of a multidisciplinary team.
• Good organisational and time management skills.
• Willing to undertake training and develop competencies relevant to the role.
• Access to a vehicle for work purposes.

Responsibilities

• Support the delivery of rehabilitation programmes prescribed by registered therapists.
• Assist patients with therapeutic exercises, mobility practice and activities of daily living.
• Provide, fit and monitor therapeutic equipment and aids.
• Monitor and report changes in patients' conditions to the relevant healthcare professional.
• Communicate effectively with patients, families and members of the multidisciplinary team.
• Maintain accurate clinical records and contribute to patient reviews.
• Promote patient independence, wellbeing and recovery.
