Job summary

East of England Community Health and Care NHS Trust in Norwich is seeking a Therapy Assistant Practitioner to support the Central Norfolk Home First Hub. The role involves triage of referrals, telephone assessments and community home visits across Norwich, South and North areas, with a 7-day rota.

You will work with an integrated MDT, maintain clinical competencies, and join a supportive team. Face-to-face interviews only are offered by this employer.

Qualifications

• NVQ 3 or equivalent knowledge and experience.
• Foundation degree programme, or equivalent demonstrable knowledge and skills.
• Good general education.
• Experience of working in a health or social care setting.

Responsibilities

• Triage therapy referrals for discharge to assess pathways.
• Conduct telephone assessments and home visits to assess needs.
• Deliver initial assessments and treatments in patients' homes or care homes.
• Maintain clinical competencies with community work and team support.
