Job summary

Norfolk Community Health and Care NHS Trust is seeking a Community Assistant Practitioner to support the Central Norfolk Home First Hub. You will triage referrals for patients on Discharge to Assess pathways, assess needs, determine care priority, and allocate referrals with the Integrated team to ensure timely responses.

• Experience with triage and care planning in community settings.
• Ability to work within a reablement philosophy and strengths-based assessments.

• Triage referrals for patients on Discharge to Assess pathways.
• Assess patient needs, plan and allocate care, liaise with the multidisciplinary team.
• Promote independence and facilitate early discharge from hospital within integrated teams.
