Job summary

Norfolk Community Health and Care NHS Trust in Norwich is recruiting a Community Healthcare Assistant to join our Community Nursing Team based at Norwich Community Hospital. You will work within a multidisciplinary team delivering high-quality, person-centred care across the Norwich community, primarily in patients’ homes and community settings.

The role offers diverse clinical responsibilities, substantial training, and a supportive induction period.

Qualifications

• Has a good range of clinical skills to manage varied health problems.
• Full training and comprehensive induction provided.
• Band 3 Health Care Assistant level within NHS.

Responsibilities

• Plan and prioritise own visits to delegated patients.
• Complete patient-held records and input on SystmOne daily.
• Make basic judgements on patients’ response to prescribed care and report to a practitioner.
• Liaise with other health care professionals as required.
• Demonstrate empathy and effective communication with patients and carers.
• Attend MDT meetings and ongoing competency training as required.
