Job summary

Sirona care & health CIC in Weston-super-Mare is recruiting a Community Support Worker to join a multi-disciplinary team delivering home-based care. You will help patients live independently, supporting rehabilitation, daily routines and personal care across the community.

The role offers a competitive Agenda for Change salary (£25,760–£27,476 pro rata per year) with full NHS benefits, generous annual leave, mileage reimbursement and opportunities for flexible working.

Qualifications

• Clear communication with service users and families.
• Ability to work autonomously in the community.
• Willingness to travel across the service area.
• Know-how to navigate UK visa sponsorship where applicable.

Responsibilities

• Support patients to live independently and improve quality of life at home.
• Deliver delegated clinical, therapy and nursing-led activities within a community setting.
• Collaborate with Physiotherapists and Nurses to meet patient goals.
