Sirona care & health CIC is recruiting a Support Worker to join a multidisciplinary team delivering therapy-based support in community settings. You will help patients regain independence and work with Physiotherapists, Occupational Therapists and Nurses, both at home and in the community.

The role requires an NVQ3 in Health & Social Care or willingness to work towards it, strong communication, and the ability to work independently as well as part of a wider MDT. Terms follow Agenda for Change.

Qualifications

• NVQ Level 3 in Health & Social Care or willingness to work towards.
• Ability to work without direct supervision within the team to achieve objectives.
• Prioritise tasks and make decisions within clear guidelines.
• Experience of direct patient care in health or social care.
• Experience with IT systems such as EMIS and venepuncture observations.

Responsabilités

• Deliver tailored therapy interventions with MDT guidance.
• Support patients in daily living and independence.
• Assist patients in setting goals and motivate progress.
• Monitor progress and report to MDT and lead clinician.
• Promote a safe, supportive environment for patients.
• Collaborate with therapists and nurses for holistic care.
• Encourage community engagement and participation.
