Job summary

Sirona care & health CIC in England is recruiting a Support Worker to join a multidisciplinary team delivering therapy-based support in community settings. You will work with patients to regain independence and improve quality of life, including home visits alongside the community nursing team.

The role requires compassionate, patient individuals with good communication, who can work alone and as part of a wider MDT. NVQ3 in Health & Social Care or progress toward it is expected.

Qualifications

• NVQ Level 3 in Health & Social Care or willingness to work towards.

Responsibilities

• Deliver tailored therapy interventions under MDT guidance with qualified health professionals.
• Support patients in developing daily living skills and independence.
• Assist patients in setting realistic goals and staying motivated.
• Monitor progress and record using EMIS, sharing feedback with the MDT.
• Promote a safe and supportive environment and encourage participation.
• Collaborate with physiotherapists, occupational therapists and nurses for holistic care.
• Encourage and enable community engagement and participation for patients.
