Job summary

British Red Cross is seeking a dedicated Support Worker for the 7-day hospital discharge service in Sandwell. The role involves helping people return home safely after hospital stays and maintaining their independence.

You will work with hospital teams and partners to coordinate discharge, within a small team that includes two other Support Workers, two Coordinators, and a Service Manager. Fixed-term contract until 31 December 2027, 37 hours per week, with a 5-in-7 rota and weekend work.

Qualifications

• Full UK manual driving licence held for a minimum of 2 years.

Responsibilities

• Help people return home safely after a hospital stay and maintain their independence.
• Work closely with hospital teams and partner organisations to support discharge.
• Be part of a team with two Support Workers, two Coordinators, and a Service Manager.
