A prominent healthcare provider is seeking a B6 nurse to join their Community Neuro Rehab Team in Brighton and Hove. The role involves delivering stroke care to community-based patients while collaborating with a diverse multidisciplinary team. Ideal candidates will have a nursing qualification and experience in patient care, particularly in stroke and rehabilitation, and should be driven by compassion and excellence in community healthcare.
The role is for a B6 nurse post of 37.5 hours a week with occasional Saturday working. Delivery of an effective stroke nurse service to community-based neurological patients - predominantly stroke. Treating all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences.
To practice as a stroke nurse in the community, liaising with the multidisciplinary team and delegating work to rehab support workers or other team members as appropriate. To assist the multidisciplinary team in preparing intervention plans, implementing rehabilitation and evaluating outcomes.
Blood pressure (BP) management is a significant part of this role, supporting patients and their families, GPs and MDT colleagues. An additional part of this role is supporting patients and their families to understand what has happened to them in terms of their diagnosis. The stroke nurse role will be expected to offer guidance and education around improving any positive lifestyle changes to empower patients to reduce their risks of further strokes happening.
To work jointly with other professionals to ensure a needs-based, patient-centred "what matters to me?" approach. We have an exciting opportunity for a nurse with an interest and the relevant experience to join our Community Neuro Rehab Team, based in Brighton and Hove.
You will be part of a large, friendly, welcoming, dynamic multidisciplinary team, delivering neuro rehabilitation to patients in a community setting. You will have supervision from the band 7 stroke nurse and work in the team with Physiotherapists, Occupational Therapists, Speech and Language Therapists, social worker, Psychologist and Rehab support workers.
