City Health Care Partnership CIC (CHCP CIC) is looking for a Community Staff Nurse to deliver holistic, evidence-based care in patients’ homes within the District Nursing Service. You will assess needs, plan care and respond to changing circumstances as part of a multidisciplinary team.

You will build strong relationships with patients, families and partner organisations, maintain accurate records and contribute to service improvement while supporting students and junior colleagues in a busy

Qualifications

  • Registered Nurse qualification and current professional registration.
  • Experience working in a healthcare setting and as part of a team.
  • Strong communication, assessment and clinical decision-making skills.
  • A full UK driving licence and access to a vehicle for work purposes.

You'll support people in their own homes through skilled nursing care, helping them stay safe, independent and well while receiving the treatment they need. The Community Staff Nurse plays an important role within the District Nursing Service, delivering high-quality care and building positive relationships with patients, families and partner organisations. Working as part of a supportive multidisciplinary team, you will use your clinical judgement to assess needs, plan care and respond to changing circumstances.

Main duties of the job

What you'll do:

Qualifications
  • Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration
Knowledge
  • Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Knowledge of the modernisation agenda
  • Demonstrate an understanding of evidence based practice
  • Knowledge of local initiatives
  • Demonstrate an understanding of the MDT
  • Basic IT skills
  • Knowledge and skills in the assessment process
