City Health Care Partnership CIC is seeking a Community Staff Nurse to provide high-quality nursing care in patients' homes, enabling independence and safety. You will assess needs, plan care and build trusted relationships with patients, families and health professionals.
You will work autonomously within a supportive community nursing team, delivering person-centred care and contributing to service improvement while developing clinical skills with experienced colleagues.
Working with a supportive community nursing team, you'll provide high-quality care to people in their own homes so they can stay as independent, safe and well as possible. Based on the role of Community Staff Nurse, you will assess needs, plan care and build trusted relationships with patients, families and other health and care professionals. You'll play an important part in delivering person-centred care, supporting people with a wide range of health needs. The role offers opportunities to develop your clinical skills, work autonomously and contribute to service improvement while being supported by experienced colleagues within the wider community nursing team.
