Job summary

First Community Health & Care is seeking a Staff Nurse to join the Responsive Services Team in the community, delivering care across Urgent Community Response, Intermediate Care, Virtual Ward and the Referral Hub.

You will assess patients at home, administer IV medications, undertake wound care, and work with a multidisciplinary team to support independence and avoid unnecessary hospital admissions. Driving licence is required.

Qualifications

• Current NMC registration.
• Registered First Level Nurse.
• Educated to degree/diploma level Mentorship, ENB 998, practice assessor or equivalent.
• Demonstrates clear evidence of on-going professional development.

An excitingopportunity has arisen for a dynamic staff nurse to join First Community Healthand Care working with the Responsive Services Team.

Responsive Servicesconsists of the Urgent Community Response Team, Intermediate Care Team, VirtualWard and Referral Hub.

This role with work in in collaboration with Primary Care andSecondary Care Colleagues, Urgent Community Response, Intermediate Care(Responsive Services), Community Nursing and other Specialist Health Care Teamsacross First Community, statutory and voluntary agencies to deliver excellentstandards of patient care for patients who are acutely unwell requiring supportto avoid admission to the acute hospital. Providing practical care, educationand training for patients and carers admitted to responsive services.

Main duties of the job

You will completeassessments in the patients home and undertake treatments including IVmedications, wound assessment, and management. You will work closely with themulti-disciplinary team to promote independenceof patients to ensure they can remain in their own home.

The candidate willbe able to adapt to change and work in a busy environment. Excellent communication skills are essentialto enable liaison with all our health and social care partners in the management and assessment in the community.

Due to the nature of the role it is essential that applicants are ableto drive, have a car suitable for business purposes and have a clean drivinglicence.
