Job summary

Surrey Downs Health and Care is seeking a Band 5 Community Nurse to join the ICP PCN teams in Ewell. The role is part-time (22.5 hours per week) with NHS AfC Band 5 grading and a pro rata salary between £33,677 and £40,996 per annum depending on hours.

You will assess care needs, develop and evaluate holistic care, support families, driving licence required, and opportunities for learning and development.

Qualifications

  • Registered Adult Nurse with NMC registration.
  • Valid UK driving licence.
  • Experience in the NHS or community nursing.

Responsibilities

  • Work as part of the team with lead responsibilities as negotiated with District Nurse.
  • Assess individual care needs, implement and evaluate holistic programmes of care.
  • Involve, support, inform and educate family/carers.
  • Promote health and wellbeing of the patient.
  • Maintain records according to SDHC procedures.
  • Mentor and supervise staff as required.
  • Ensure timely patient care and data entry.

We have an exciting opportunities for a Band 5 Nurses to work at ICP PCN, within Surrey Downs Health & Care. If you are passionate about caring for people and making a difference within your community, we would love to hear from you!

We are looking for nursing staff from all backgrounds including newly qualified, those more experienced, looking for a change, maybe wanting to work locally, excited by the difference of being out within the community with the benefits and challenges that brings.

To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.

Please note these roles require post holders to hold a current and valid driving licence enabling them to drive in the UK and have access to a vehicle for work purposes.

We have our own Learning & Development Department that have a range of different courses and development plans that give you the opportunity to grow your career.

Main duties of the job
  • Work as part of the team undertaking lead responsibilities as negotiated and agreed with District Nurse
  • Treat all patients as individuals, respecting their privacy and dignity at all times
  • Assess individual care needs, implement and evaluate holistic programmes of care
  • Involving, supporting, informing and educating family/carers
  • Take every opportunity to promote health and wellbeing of the patient
  • Develop effective communication and liaison systems
  • Maintain clear and comprehensive, signed and contemporaneous records according to SDHC procedures
  • Facilitate/undertake training and professional updating for team and students
  • Act as an assessor and mentor to qualified and unqualified staff
  • Maintain, develop and monitor standards of evidence based nursing care
  • Maintain own professional and clinical integrity in line with NMC guidelines
  • Support/mentor team members and participate in clinical supervision activities
  • Work without direct supervision and to maintain close liaison with the senior team member, deputising as required
  • Be responsible and maintain timely patient care
  • Be responsible for maintain data entry and other record keeping
  • To work in co-operation and partnership with all other professionals and agencies involved in the care of the patient
  • Be aware of and act upon when necessary, procedures that are in place to protect vulnerable individuals
