Job summary

Surrey Downs Health and Care is recruiting Band 5 community nurses to work across ICP PCN in the Surrey Downs area. The role is part time (22.5 hours per week) at Bourne Hall Health Centre, Ewell, with a pro rata salary. You will assess care needs, develop and evaluate holistic care plans, and work with a multidisciplinary team.

A valid UK driving licence and access to a vehicle are required. We support professional development through our Learning & Development Department and offer

• Registered Adult Nurse with NMC is required.
• Valid Driving Licence (to drive within UK) is required.
• Experience in community nursing and NHS context is desirable.

Responsibilities

• Assess individual care needs, develop and evaluate holistic programmes of care.
• Lead responsibilities as negotiated with District Nurse.
• Maintain clear and comprehensive records according to SDHC procedures.
• Support/mentor team members and participate in clinical supervision.
• Work with other professionals and agencies involved in patient care.

To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.

Please note these roles require post holders to hold a current and valid driving licence enabling them to drive in the UK and have access to a vehicle for work purposes.

• Work as part of the team undertaking lead responsibilities as negotiated and agreed with District Nurse
• Treat all patients as individuals, respecting their privacy and dignity at all times
• Assess individual care needs, implement and evaluate holistic programmes of care
• Involving, supporting, informing and educating family/carers
• Take every opportunity to promote health and wellbeing of the patient
• Develop effective communication and liaison systems
