Job summary

The post holder joins the Calderdale CAMHS Learning Disability Team within South West Yorkshire Partnership NHS Trust to provide community-based nursing care for children and young people with learning disabilities.

You will conduct holistic assessments, contribute to care planning, and work with multi-disciplinary colleagues and partner agencies to support transitions to adult services within a supportive community setting.

Qualifications

• RNLD registration is essential.
• Teaching qualification is essential.
• Evidence of additional academic study at Diploma/degree level.

Responsibilities

• Carry defined caseload within the community learning disability team.
• Lead multi-disciplinary meetings and plans for care reviews.
• Collaborate with partner agencies and specialist schools.
• Advise and support families to meet children's health needs.
