Qualifications
- RNLD or suitably experienced nurse with current NMC registration.
- CPD and ongoing professional development evident.
- Experience in supervision or mentoring.
- Experience with learning disabilities and complex health needs.
Responsibilities
- Provide person-centred assessment and MDT-informed clinical interventions.
- Support a defined caseload of adults at risk of placement breakdown.
- Work with IST colleagues and other services to improve local LD services.
- May include planned out-of-hours work and on-call support.
