The Children’s Community Learning Disability team in South Staffordshire are expanding and have an exciting opportunity for a creative and innovative Specialist Nurse who has experience of working with children who have a learning disability and their families. In the role you will have the opportunity to work alongside an experienced group of Learning Disability Nurses, Psychologists, Occupational Therapist, SLT and Psychiatrist.
In MPFT Children and Families Mental Health Function, we work with young people who have a wide range of needs using an inclusive, person-centred approach. Our model places our young people’s needs at the heart of what we offer to ensure that they are getting the best care possible. The Children’s Learning Disability team offers: support and intervention for young people with emotional and physical healthcare needs who may be at risk of placement breakdown or hospital admission.
You will be responsible for the management of a complex caseload, which
includes, specialist assessment (within service specification), care planning in
conjunction with service user and carer, evaluation of individual treatment
plans for CYP with learning disabilities.
