Job summary

Coventry and Warwickshire Partnership NHS Trust is seeking an experienced RNLD to join the Community Learning Disabilities Team in North Warwickshire. The post involves managing own caseload, collaboration with multi-disciplinary colleagues in the community and delivering specialist nursing advice to adults with learning disabilities and their carers.

Responsibilities

  • The post holder is responsible for specialist assessment of client needs, including those with complex needs.
  • Develop, implement and evaluate programmes of care for the caseload.
  • Act as health facilitator for people with a learning disability.
  • Promote needs/rights of people with learning disabilities to other agencies and the public.
  • Collaborate with multi-disciplinary colleagues in the community and agencies.

Community Learning Disabilities Nurse

An exciting opportunity has arisen for an experienced RNLD to work as part of a Community Learning Disabilities team in North Warwickshire.

The role involves close collaborative working with multi-disciplinary colleagues in the community setting, managing own caseload, Supervising others, completing assessments, raising awareness of the health needs of adults who have a learning disability, identifying unmet health needs and providing specialist nursing advice.

The successful candidate will need to be proactive, self-motivated and have experience of autonomous working, whilst also working under the direction of the community team manager.

The successful candidate will also be able to use current best practice to gain the best outcomes for Clients and their Carers. Will also assist in improving links with primary care services including hospitals/ G.P surgeries, provide support, information and education to clients, carers and provider's and also deliver education to people with learning disabilities around health needs both on 1:1 or group basis.

Essential Requirements include:

  • Substantial post registration experience as a Band 5/6 nurse working with people with a learning disability
  • RNLD and current NMC Registration
  • Experience of working with people with a learning disability in the community
  • Driver with access to independent means of transport
Main duties of the job

The post holder is responsible for the specialist assessment of client needs, including those with complex needs and/or behaviours that challenge, the development, implementation and evaluation of programmes of care.

The post holder will have responsibility for a caseload.

The post holder will also act as a health facilitator for people with a learning disability.

Job responsibilities

To promote the needs and rights of people with learning disabilities to other agencies and the general public.

To identify and evaluate specific client needs through specialist assessment.

To assist in the development of new services and projects within the community to meet identified clients needs, including those with complex needs and/or behaviours that challenge.

To participate as an active member of the Learning Disability Nursing Team and establish and maintain effective communication with other agencies and professionals.

To act as a health facilitator for people with a learning disability.

To report as soon as possible to the Team Manager any difficulties that may arise.

Qualifications
  • Diploma / Degree holder
  • Has NMC
Experience
  • RNLD with a substantial post registration experience at Band 5 or Band 6 working with people with a learning disability
  • Experience of working in multi agency teams with multi-disciplinary colleagues
  • Experience of working as a community nurse
  • Experience and knowledge of challenging behaviour
Other
  • Independent means of transport and UK driving licence
