Job summary

Responsibilities

• Assess, plan, implement and evaluate care for intermediate care patients.
• Provide rehabilitation and support for step down/step up patients.
• Supervise students and junior colleagues where appropriate.
• Coordinate discharge planning with multidisciplinary teams.
