Job summary

Northern Lincolnshire and Goole NHS Foundation Trust is seeking a Band 5 Nurse to join the Intermediate Care team in North Lincolnshire. The post focuses on short-term rehabilitation and discharge planning, working with social care and GP teams to help patients return home.

You will support a range of settings across North Lincolnshire and be involved in developing community nursing practices, with a strong emphasis on communication, teamwork and ongoing professional development.

Qualifications

• Registered Nurse with current UK registration.
• Evidence of post-registration development is desirable.
• Desirable degree in nursing or related field.

Responsibilities

• Undertake comprehensive and holistic assessment of the patient’s nursing care needs for complex patients within Intermediate Care.
• Develop a plan of care in partnership with the patient, based on assessment findings.
• Implement the care plan, delegate duties, and coordinate other healthcare team members, patient, family and carers.
• Evaluate and adjust the care plan based on patient condition and feedback to caseload holder.
• Advise on health promotion and illness prevention as part of nursing duties.
• Maintain clinical records in accordance with NMC and Trust policies.
