Job summary

North Cheshire and Mersey NHS Foundation Trust’s Band 5 Intermediate Care Nurse role will be primarily based on Ward B3 at Halton Hospital, supporting the Community Winter Stepdown Ward from 1 December 2026 to 31 March 2027.

The ward provides rehabilitation and recovery-focussed care for patients after an acute stay or as an alternative to admission, with potential redeployment into other community nursing services.

Qualifications

• Assess, plan, implement and evaluate patient care in line with rehabilitation goals.
• Deliver high-quality nursing care for patients requiring intermediate care and rehab.
• Promote patient independence and personalised care planning.
• Coordinate care with the multidisciplinary team for discharge planning.
• Administer medications safely per Trust policies.
• Maintain accurate patient records and documentation.
• Support education, health promotion and discharge planning for patients and families.
• Participate in ward quality improvement, audits and service development.
• Act as a patient advocate, delivering care with dignity and respect.
• Supervise healthcare assistants and junior colleagues.
