Job summary

Epsom and St Helier University Hospitals is recruiting a Rehabilitation Assistant to join the Care Home Support Team in Carshalton. You will work across care homes to deliver person-centred rehabilitation, under supervision of physiotherapists and occupational therapists, focusing on mobility, independence and quality of life.

The role offers full-time hours (37.5 per week) with a band 3 salary of £30,630–£32,346 per annum pro rata in outer HCAS.

Qualifications

• Ability to support assessment of mobility, transfers and functional ability.
• Experience in delivering delegated rehabilitation programmes with supervision.
• Knowledge of continence assessment and equipment handling.

Responsibilities

• Deliver delegated rehabilitation programmes in care homes.
• Support continence assessments and plans.
• Work in MDT with therapists and nurses to support residents.
• Assist with mobility aids and equipment and falls prevention.
• Coach care home staff to promote mobility and independence.
• Manage delegated caseload and respond to changes promptly.
• Document accurately in electronic records and support quality improvement.
