Job summary

South Tees Hospitals NHS Foundation Trust Community Collaborative and Primary Care Network are recruiting a Band 6 physiotherapist to join the integrated community Therapy team. The role is 37.5 hours per week and will involve rehabilitation in patients' homes and group settings across Redcar, Middlesbrough and surrounding areas.

You will work closely with statutory and voluntary services to support the Teeswide Falls strategy and initiatives to keep people independent at home, with travel

Qualifications

• BSc or Diploma in Physiotherapy and HCPC registration.
• Postgraduate experience across a broad range of clinical specialties.
• Experience working within NHS and in a multidisciplinary team.
• Willingness to travel across Redcar and Cleveland locality.

Responsibilities

• Provide rehabilitation in patients' homes and group settings.
• Work with urgent community response to step/step down patients.
• Promote independence and coordinate with statutory and voluntary services.
• Support development and attend postgraduate courses.
