Job summary

South Tyneside and Sunderland NHS Foundation Trust is recruiting a Highly Specialist Physiotherapist to join the Urgent Community Response Service in the Recovery at Home Team across Sunderland. Based between Leechmere Independent Living Centre and the Clarendon Care Coordination Hub, the role leads advanced assessment and home-based rehabilitation for patients with acute illness or post-discharge needs.

Qualifications

• HCPC registration is required and ongoing professional development is expected.
• Qualified physiotherapist with BSc or MSc in Physiotherapy.
• Experience in urgent/community rehabilitation preferred.
• Ability to lead clinical development and supervise staff.

Responsibilities

• Lead the Recovery at Home Team delivering rapid home assessments seven days a week.
• Undertake autonomous assessments, develop treatment plans and initiate appropriate interventions.
• Coordinate triage and prioritise calls to ensure timely responses.
• Provide expert physiotherapy and mentor junior staff within the MDT.
• Support service development, quality improvement, audits and implementation of best practice.
• Promote innovative, cost-effective community care delivery.
