Job summary

Somerset NHS Foundation Trust is seeking a physiotherapist to join the Pathway 1 (D2A) team within the Intermediate Care service in Taunton & West Somerset/Sedgemoor. You will assess patients in their own home to plan rehabilitation and equipment needs, coordinating care across health and social care with a strong emphasis on independence and preventing hospital admissions.

You will work within a supportive multidisciplinary team, pursuing professional development and contributing to service

Qualifications

• HCPC registered and eligible to practice in the UK.
• Full driving licence or access to own car is desirable.
• Experience in community rehabilitation and/or acute care settings.
• Experience with frail and elderly population and providing rehabilitation.
• Evidence of advanced clinical reasoning.

Responsibilities

• Carry out therapy assessments including goal setting and treatment planning.
• Assess risk and promote independent functioning.
• Work autonomously and as part of a multidisciplinary team.
• Coordinate care across health and social care services and provide clinical advice to colleagues.
• Contribute to service development and quality improvement.
