Job summary

Colchester Hospital University NHS Foundation Trust is seeking a Band 6 Community Physiotherapist to join the North East Suffolk team delivering high-quality care in patients' homes. You will triage referrals, manage complex caseloads, and develop tailored treatment plans with a focus on self-management and patient empowerment.

The role requires excellent communication, collaboration across professional boundaries, and a valid driving licence for community-based work.

Qualifications

• Qualified Physiotherapist with HCPC registration.
• Experience at Band 6 level in community settings preferred.
• Must hold a valid driving licence for community-based work.

Responsibilities

• Triage and schedule referrals to the service in line with Trust policies.
• Manage a caseload of patients with complex needs.
• Perform assessments and develop treatment plans within holistic care.
• Create individualised management plans using advanced assessment and clinical reasoning.
• Ensure interventions are evaluated and discharge plans are timely.
• Provide training and advice to patients and carers to support self-management.
